The multiple choice 'Performance Diagnostics' questionnaire is a hand coded PHP form, presenting instant results to questions in both categorised and overall percentages.
Entirely hand coded in XHTML, CSS and PHP; the website was later developed into a Wordpress Theme and the Wordpress CMS was integrated to provide Turnbull Management Ltd with control over their own content.
